Purpose

These forms are to document maintenance and use of U. S.
government-owned property used with grants/contracts.

NOTE: These forms should be attached to the equipment, if
possible.

The principal investigator is responsible for assuring government
property is used only for contract/grant authorized purposes and
for maintaining the required utilization records and maintenance
records.
